California 4-H strives to provide healthy and positive environments that help members reach their full potential. The role and performance of a chaperone can significantly influence the positive outcomes of youth participation at these events.

Chaperones who are selected for state and national events are expected to serve as a representative and partner of the California 4-H State Office and participate in all scheduled activities as prescribed by the selected event.

Please complete this form to apply to be a chaperone at any State or National event. Application does not guarantee acceptance. We choose chaperones based on registration needs, experience and ability to meet requirements and essential functions and county personnel endorsement.
