PARD's Derby 101 Program Registration
Complete this form to register for PARD's Derby 101  program.  Please note that the first three weeks will be focused on learning to skate (we will teach you from the ground up - literally!)

Registration fee of $150 can be paid by cheque, cash (in person), or by e-transfer to ptboareard@gmail.com (please make the answer to the security question the word "rollerderby" and add your human name in the message field).

You must also purchase WFTDi insurance, the link will be at the end of this registration form.
