Incident Form
This form is an avenue for notifying leadership of an issue or complaint regarding the conduct of community members and event attendees. This information is used by our Board and designated volunteers to evaluate and determine an appropriate course of action.

While we appreciate a desire to remain anonymous, providing your contact information allows us to follow up with you to resolve questions and to provide a status on the actions we have taken. We store this information in a database visible only to select OPEN Northwest volunteer staff who have been trained to handle Complaints and Conduct issues.
