Student Walk/Bike Home Parent Consent and Liability Waiver
Dear Parents/Guardians,
We understand that some students prefer to walk or bike home after school. In order to ensure the safety and security of your child(ren) while allowing them this option, we require parental consent. Please complete and sign the permission form below to grant your child(ren) permission to walk or bike home from JKL Bahweting School. By signing this form, you agree to release and absolve JKL Bahweting School, its employees, administrators, and affiliates from all liability in the event that your child(ren) is/are injured or involved in an accident while walking or biking home. 

I, the undersigned, grant permission for my child(ren) listed below to walk or bike home
from JKL Bahweting School on a regular basis or as otherwise indicated. I understand
and acknowledge that:

1. My child(ren) will be dismissed from school and will be responsible for their own
safety and behavior once they leave school grounds.

2. JKL Bahweting School, its staff, and affiliates are not responsible or liable for any
injuries, accidents, or other incidents that may occur while my child(ren) is/are walking
or biking home.

3. I agree to release, discharge, and hold harmless JKL Bahweting School, its employees,
administrators, and representatives from any claims, demands, actions, or causes of
action arising out of or related to any injury or damage that may occur while my
child(ren) is/are walking or biking home.

4. I understand that this permission and liability waiver applies for the duration of the
school year, unless I notify the school otherwise in writing.
