16. Dogs may be in foster care anywhere from a few weeks to a few months, depending on how long it takes to find a good match for the dog.  Are you able to make this commitment? *

20.  As an organization that relies on foster homes to care for our dogs, do you understand that we do NOT have a physical shelter for the dog to be returned to if you decide you no longer want to foster the dog anymore?  If you decide you do not want to foster a dog anymore, we will need at least a few days to find a new foster home for him or her? *
21. As a dog foster, you may have to take the dog to and from our vet, events, or pick-up supplies such as food.  We will try to accommodate as much as possible, but are you willing to help with these tasks if needed? *
22.  If you have a vet, please provide one vet reference with phone number and which name the pet's account is under (we check to make sure your pets are spayed/neutered and have been up to date on vaccines and heart worm and flea/tick preventatives). *
23. Please provide a personal reference's name (not a significant other or close family member) and phone number.  Please let your reference know we will be contacting them as part of the screening process. *
24.Please provide your landlord contact information (name and phone number), if you do not own. *
