Internship Evaluation Form
In order for students to develop professionally and to provide students with a final grade for their Internship experience, Supervisors are required to submit the following Internship evaluation form monthly throughout the Internship experience. The form should be submitted once in each of the following months for a total of 5 submissions: February, March, April, May, June.
