This form needs to be completed if you are MOVING OUT of the Village or MOVING IN to the Village.
NOTE: If you are relocating within the Village, submit one form for the address you are leaving and another form for the new address you will be moving into.

MOVING OUT: You would like the utility bill out of your name and need a Closing bill.  Please have your account number available as this information helps expedite your request.
NOTE: This is a billing request only.  This submittal does NOT cause the water to be shut off at the service address.  You must contact Public Works to have the water shut off.

MOVING IN: You would like the utility bill in your name.
