Attendance Note

This form is to be filled out and submitted only by the student's parent or legal guardian.

Please refer to our handbook for the attendance policy.  

Use this form to submit one of the following:

  1. Early Dismissal- must notify before 8:30 AM. After 2:15 PM early dismissals are not permitted. 
  2. Late Arrival
  3. Absence 

Then, send a quick note via Class DOJO to notify the teacher.
