The registration fee is $50.00 a team (a team consists of 2-3 participants).
- There is a $5.00 discount per team, if you have registered on or before Spring Training (18 January 2020.)  
- There is also a $5.00 discount per team if the COACH attends Spring Training.
The registration fee funds the event. There will be a voluntary offering taken during the event for Bruce and Marti Brumfield (our Awana Missionaries) to fund their on-going work in Northern Illinois and Middle Tennessee.

This form consists of 3 sections:
- Section 1 Church, Contact and Bible Version
- Section 2 Team Information and Location
- Section 3 Payment Information

Remember to provide two volunteers to help staff the event.

You will be able to re-edit this form after it is submitted. You will also receive a copy of this form by email for your receipt. Please note that payment this year is by check and not by credit card.  Make Checks Payable to Lake County Baptist Church and send them to Lake County Baptist Church, 1550 Yorkhouse Rd, Waukegan, IL 60087 (Put "Awana Event" on the check.)
