CMWHL DEK HOCKEY IS BACK!
Games will be held every Tuesday evening at 7:10pm & 8:10pm, beginning on 9/9/25, at Fitchburg Street Hockey Rink. Each team will also play TWO SUNDAY MORNING games this fall. The regular season will run for 10 weeks, and playoffs will immediately follow. 
This is a DRAFT LEAGUE. You will get an invite from TeamLinkT once the teams are made. You will need to download the app to see the schedule and team info.

CENTRAL MASSACHUSETTS WOMEN'S HOCKEY LEAGUE PARTICIPATION AGREEMENT INDEMNIFICATION AND RELEASES  
In consideration for Participant's ability to participate in the Central Massachusetts Women's Hockey League (“CMWHL"), including any and all activities related to practices or training sessions, coaching, playing games,  performances and competitions, and traveling to and from any of the function activities (hereafter referred to  as "Sport Activities"), the Participant signing this Agreement agrees as follows.  
1. Requirements: Each Participant is required to meet the following requirements  

(a) Any participant under 18 years of age must have the Board approval and written consent from Parent or Lead Guardian  

(b) Participant warrants that she is in good health, that she has consulted with her doctor with regard to participation in Sport Activities and that there is no medical condition which would prevent her participation  

(c) Participant must pay a non-refundable participation fee of $105 (subject to change) for Team Members  Said fee shall be forfeited if Participant either terminates her participation or is dropped from the team for any reason:  

2. Medical Condition: The Participant understands the nature of the sport of hockey and the inherent or potential risks associated with hockey and all Sport Activities. The Participant warrants that she is in sufficiently good health and physical condition to participate in all Sport Activities, that she has consulted  with her medical doctor and has been found to be healthy enough to participate in Sport Activities, and that  she voluntarily wishes to participate in such activities. It is the Participant's responsibility to insure that she has proper medical insurance coverage for all Sport Activities. The CMWHL assumes no liability or obligation  arising from any actual or alleged failure of a Participant to consult with a doctor or for any injury, damage,  loss, costs of expenses related to or arising from participation in Sport Activities.  

3. Code of Conduct: Participants shall comply with instruction and directions of Team Activity coaches, leaders and instructors, and with the Team’s Code of Conduct. The Participant shall conduct herself at all times in keeping with high moral and ethical standards so she reflects positively on herself and the Team. Failure to  meet these obligations may, at the sole discretion of CMWHL, result in immediate removal from Sport Activities and/or being dropped from the Team and being barred from any future participation.  

The Participant shall,  
1. Act in the spirit of fair play and true sportsmanship  
2. Refrain from using offensive language  
3. Be fully committed to Sport Activities, including arriving on time for games and following the instruction of the CMWHL board members, guest instructors, and referees.  
4. Treat all people with respect.  
5. Promote the well-being of teammates by positive counseling and encouragement or by reporting any  unhealthy or dangerous conduct to coaches.  

The Participant shall not,  
1. Engage in disrespectful conduct including the area of profanity, obscene gestures, and offensive remarks, horse playing, taunting, or other actions that demean individuals or the sport  
2. Fight or show excessive display of anger or frustration.  

Any misbehavior will not be tolerated.
The Participant will be asked to leave and not return until they can obey the Code of Conduct.
No refunds will be given for a Participant who has been asked to leave for violating the CMWHL Code of Conduct.  

I have read and understand the requirements of this Code of Conduct. I understand that I am expected to perform according to tins code and I understand that there may be sanctions or penalties if I do not.

4. Potential Risk: The sport of hockey contains potential risks for harm or injury, including injury that may lead to permanent and serious physical disability, paralysis, brain injury, or death ("Injuries”). Injuries might  arise from the participation in the sport of Hockey itself, or from other related, consequential or ancillary  actions and events related to Sport Activities. All such risks are deemed to be inherent to the Participant's  participation in Sport Activities. By this Agreement, the Participant is deemed to fully assume all such risks and, in consideration for the right of the Participant to participate in Sport Activities, understands and agrees that to the fullest extent allowed by law, she is waiving and releasing any potential future claim she might  otherwise have against  the CMWHL  or any employee, agent or volunteer of CMWHL, by or on behalf of herself, or any administrator, executor, trustee, assignee or family member of the Participant.  

5. Emergency Medical Care. If an injury or medical emergency occurs during Sport Activities, Participant  hereby gives her express permission to  CMWHL employees, agents or volunteers to administer or authorize at  the administration of urgent or emergency care, including the transportation of the Participant to urgent care or emergency care provider. If Participant has any serious allergies or other medical conditions that  emergency care providers should know of, Participant should wear a medical alert bracelet with the pertinent  information. All costs and expenses associated with such care are solely the responsibility of the Participant.  

6. Promotional Release Authorization. Participant hereby agrees that her likeness, name and other information may be published or reproduced in public media in connection with her Participation in Sport Activities.  

7. Governing Law. This Agreement shall be governed by the laws of the Commonwealth of Massachusetts.  This Agreement is to be broadly construed so as to enforce the purposes set forth above and shall not be  construed against either party. If any part of this Agreement is deemed invalid or ineffective, all other provisions shall remain in force. No oral modification or change of this Agreement is allowed. This Agreement contains the sole and exclusive understanding of the parties, and the Participant has relied on any oral statement  or representations in determining whether to execute this Agreement or to participate in Sport Activities.  


Payment
FULL-TIME PLAYERS ($100 per season)
Players may pay in full at the time of registration, or separate into two equal payments as follows:
$50 by 9/9/25 and $50 by 10/7/25. 

Failure to pay by the above dates will result in being ineligible to play.

SUBSTITUTE ONLY 

If you are playing as a substitute,  the fees are as follows and due on arrival at the rink.
 
$5 per game
