This application will take approximately 20 minutes to complete.
There is no ability to save this form. Once you begin you will need to complete the form.
Prepare the following information before you begin this form:

1) Social Insurance Number
2) Full mailing address including postal code for your main home
3) Full parent/guardian name and their home/cell number
4) You should be prepared to answer questions about your intended plans for next year
5) The name of the school and the program name of any post-secondary school you may attend
6) Family members who were or are military veterans
7) Family members who are NSTU members (ex. teachers, administrators, staff)
8) Family pre-tax income
9) Number of dependents living in your home
10) The names and dollar amounts of bursaries or scholarships you have already won
11) Whether you are applying for a student loan
12) List of extra-curricular activities you have participated in while at school

Citadel High provides 40+ internal scholarships to our graduates each year at graduation. These internal scholarships are provided through generous donations by members of our community. These donors decide on the criteria. The Citadel High Scholarship and Awards Committee then select the winner of the award from the responses provided in this form. BY COMPLETING THIS FORM ONCE, YOU WILL BE CONSIDERED FOR ANY AWARD FOR WHICH YOU QUALIFY.

THIS APPLICATION WILL PERMANENTLY CLOSE AT 11:59pm ON SUNDAY, MAY 6th, 2018.
Potential graduates who have not completed this application by this time will not be eligible or considered for internal scholarships or awards this year.
