To file a report with the FirePony Creative Society (FPCS) Conduct Committee (CC), please fill out and submit this Incident Form. This form collects information regarding possible Code of Conduct violations. The CC reviews credible reports of misconduct incidents that took place at our events or have community wide impact. The completed form should contain sufficient detail for the committee to begin an investigation.
Incidents may be categorized as Severe, Serious, or Moderate. The following are provided as examples and are not definitive or intended to encompass all possibilities - any violation could be considered more or less serious depending on context and potential for community-wide impact.
● Severe violations include: physical assault, sexual assault, abuse / endangerment of a minor, non-consensual intoxication (i.e. dosing), possession of a firearm
● Serious violations include: consent violations, intimidating or threatening behavior, boundary violations, verbal abuse, harassment, hate speech, misuse of position, over intoxication resulting in endangering self or others, vandalism resulting in significant property damage
● Moderate violations include: trespassing, vandalism w/out significant property damage, possession of fireworks, taking photos/video without permission, disregard of Leave No Trace policy, misappropriation of artistic or intellectual property, social media activities
