Stern MASS Prior Student Records Request
Student information is considered confidential and cannot be disclosed without the written consent of an authorized person: legal parent/guardian or student that is eighteen years or older. To ensure confidentiality of student information, this form is only valid when completed by an authorized person. A valid photo ID is required upon submission of this request. All requests require at least 5 business days for processing. Diploma replacements take 4-6 weeks for processing.
 
You are able to request the following documents:

- Unofficial Transcripts - Issued for student personal use only
- Official Transcripts - Official student academic record that includes school seal, accreditation seal, and administrator signature.
- Cumulative Records (copy only) - Student academic progress and attendance, used in transition of a student to another school.
- Diploma Replacement ($25/diploma) - Diplomas takes 4 to 6 weeks upon payment to order.

A valid government-issued photo ID as confirmation of your identity is required to start this request. Identification will be verified with school records at the time of attendance.
