The Oncology Care Model (OCM) team invites you to attend a public forum open to all interested OCM stakeholders. The Oncology Care Model Stakeholder Feedback Meeting will take place on Thursday, May 3, 2018, from 1:00 PM – 4:00 PM EDT.
The goal of this forum is to hear stakeholder feedback about OCM. The meeting will include a brief introduction from the OCM team followed by discussion.
If planning to attend in-person, we suggest registering as soon as possible. In-person attendees are on a first-come, first-serve basis. In-person registration will close one week in advance of the forum; however, foreign nationals must register for in-person attendance by Monday, April 9, 2018, due to security reasons. Persons who are not registered will not be permitted to enter the CMS Central Office and thus will be unable to attend the forum. Space is limited; register by Thursday April 26, 2018.
If attending in-person, please arrive at least 15 minutes early for check-in and security purposes. A CMS team member will be posted at the main entrance to assist you. State-issued identification, preferably a driver’s license, will be required. If proper identification is not provided, access will be denied. Additionally, all vehicles will be subject to a vehicle inspection prior to entering the complex.
Call-in information will be provided to all registrants in the registration confirmation email.
CHANGE IN PLANS?
If your plans change and you will not be able to attend in person, you will be able to change your registration from in-person to virtual. It is important that you edit your attendance preference to ensure space for all attending in person and to provide you with a link to the virtual event. After registering, you will receive two confirmations; one from Google Forms, and the other from OCM Support.
