We encourage student-athletes to ride the team bus back from away games, but understand there may be times when parents/guardians need to drive their child home. In those rare instances, please complete this form.

This form must be submitted by 3:00 pm on the day of the event.

Once submitted, the Athletic Department will notify the head coach.

Please also communicate directly with your head coach that this form has been submitted.

State law permits only parents/guardians to transport their child.
