You MUST HAVE a COMPETED PlanetHS account, physical and all other documents completed by Friday, April 30th. You will NOT be permitted to tryout if all accounts and forms are not completed by this deadline. *

Physical and planetHS documents can be accessed by following these steps. Go to dyercs.net. Click on Menu at the top. Select School Nurses. Click on health documents. You can access and print the sports physical form and the instructions for PlanetHS registration. School nurses have paper copies of these documents if you would prefer to pick one up. Each candidate is responsible for having the appropriate forms. *

Please read the financial obligations carefully. Approximately $375 will be due by May 31st. Approximately $150 will be due August 1st. We will fundraise to help with the August balance. *
