3.50CARES free meal/care package request form
We deliver FREE care packages which include a full meal (main course, side dish, and salad), snacks (fruit and crackers), and sanitary pads (for women). If you are homeless we will provide you with hand sanitizer. If you are NOT homeless and/or have access to running water, we will provide one bar of soap per household. We can provide a meal/care package for each person in your family. We should clarify that we do NOT provide groceries. Please fill out this form in it's entirety and we will be in touch with you as soon as possible with a dropoff date/time. To promote our page, help accrue donations (which help feed many more people), and help to find individual sponsors for everyone in need, 3.50CARES will film deliveries and may share your story/photo/video on social media and their website (your name, address, and telephone number will NOT be shared publicly). *
I understand that 3.50CARES does NOT provide groceries, is NOT responsible for providing more than what is stated in the description at the top of this page, and will NOT provide hand sanitizer to those with access to running water/NOT homeless. I also understand that response time may not be immediate due to a large volume of requests. If our team determines that you are in immediate need of our services, then within 48 hours of this request form closing, we will contact you via text or call to confirm your interest in the week's meal, and will then set up a delivery date/time. If our team determines that you are NOT in immediate need of our services, then you will NOT receive any further communication from us (but you may still fill out this request form at a future time). Please respond ASAP to all communication from us so we can better serve both you and many others. *
