Fill out the following form for each child that wishes to join the 2022 Summer Reading Program. Only complete this form if you plan to participate in the SR program completely online.

This form registers your child for the program and lets us know to get a registration packet prepared for them. After registering, call the Children's Library to schedule a curbside pickup to get your registration packet. (847) 587-0198
