You have the option to submit feedback with your name or anonymously: the form will not collect your email address or any other information that you do not supply.

Please note that if your feedback is submitted anonymously, there will be no way for us to contact you to let you know how your concerns are being addressed, but all feedback will be read and discussed, and we will try to find the best way to address your concerns.

Clearwater's philosophy of responding to serious concerns is outlined in the Whistleblower Policy which can be found in our rulebook linked on our website. Responses to this form are visible to the Staff and Kendra, and may be shared with JC or SM. Responses that are malicious, joking, or knowingly false may be ignored.
