2025-2026 Virtual Event RSVP and Permission Waiver Form: K-2 Virtual Trip to the Moon
The Parent/Legal Guardian of attendees to a Sanctioned Virtual Event must read and fill out this form in order to participate in the Sanctioned Virtual Event. This form is a release of liability and a waiver of certain rights.

This form must be submitted before attending the Sanctioned Virtual Event and acts as your official RSVP to the following Event:
