Transfer Request Process:

1. Parent has conversation with PM about transfer.

2. Parent fills out this Google Form and notifies their Program Manager.

3. Once transfer decision is made, you will receive an email with next steps from the Senior Director of Programs.

NOTE: It is one of the Horizons for Youth policies (5.2.1) that “An individual family’s desire to change schools within the HFY program will only be considered under compelling circumstances and requires Executive Director approval.” The Horizons for Youth Parent contract also states: "Communicate desires or plans to transfer schools or move outside of Chicago in advance via the Horizons for Youth school transfer form; understanding that an unapproved school transfer or move out of Chicago will make my family ineligible for the program."
