This application should be filled out by either the CBA church pastor and/or the church plant resident requesting the church planting residency grant. Please fill out each required item to the best of your knowledge so that the CBA Church Planting Team can discern the approval of the grant. Not all applications will be accepted, but feedback will be provided for any rejected grant applications as the funds are limited.
