Please complete the following information. If you do not know the EXACT date of the Baptism, please just guess and indicate that in the comments. Candidates not baptized at Our Lady of the Lake must have a newly issued and stamped Baptismal Certificate mailed directly to Our Lady of the Lake (not the parents). Certificate must be dated within six (6) months of Confirmation ceremony. (Must be requested on 12/12/2018 or later.) Unless you indicate otherwise on the bottom of this form, Our Lady of the Lake will request the baptismal certificate for you.
