* Date of Birth, Gender and any medical conditions
* Your home address, including postal code as well as a home phone number and
cell phone number
* Citizenship and Residency Information - including: Country of Birth, Country
of Residence before arrival in Canada, Country of Citizenship, Arrival Date in
Canada, Status in Canada, Proof of Status Document, Languages Spoken
* Emergency Contact Information
* Educational Background (report cards, translated transcripts etc.)
* If you have not previously attended Adult Day School you will need to provide
a copy of your most recent transcript and identification (proof of residency
and address)
