DAYHA Scholarship Application 2022-2023
Please fill out this application for EACH hockey player in the household who is seeking financial assistance.

You may apply at any time. As funds become available we will distribute them as quickly as possible.
The intent of our scholarship program is to off-set a member’s registration fees by applying some amount of award toward the registration fees for the upcoming season. The exact number and amount of scholarships awarded in a given year is contingent upon the success of the association’s fundraising efforts and must be approved by the DAYHA scholarship committee. Applications received by the end of a calendar month will receive a response by the 15th of the following month.
