Please submit this form to request approval to ride the shuttle bus. To be eligible students must be zoned to ride a school bus to and from school. Students must also not have extensive disciplinary issues. LIMITED SPACE. All middle school students should be picked before 5:30 except on special days when the facility closes early. Please note that due to the Youth Center being on FPC's campus 95% of the students attending will be high school students.

Please allow a few days for request to be processed. Students that are approved will be given a pass from the Buddy Taylor Dean.
