Student Housing Work Order Form
This form allows you to submit a maintenance request regarding your Student Housing unit. Please fill out the form in its entirety and follow any prompts provided to you.

By filling out this work order, you understand that members of Housing or Facilities services staff will enter your unit one or more times to diagnose and repair any issues associated with your work request. This may occur when you are not at home. Staff members will always knock twice and announce themselves before they enter your unit, giving you time to answer the door if you are home.

Please note: for emergency requests related to cold units, main unit door lock and latch issues, non-functional appliances, or plumbing issues, please contact the RA Emergency Cell Phone immediately:
