Parent School Survey
Community Investment Corporation, a local Arizona nonprofit, is working to create a tool for families to learn more about all the schools in Arizona. Our first step in the process is learning from you.   This particular survey is intended for parents or guardians of children who are currently enrolled in Preschool - 12th grade.

Complete this 10-minute (13 questions) survey and be entered to win a $50 VISA Gift Card. Please complete the survey to be entered in the drawing.
