After submission, you will need schedule an appointment to come into the Administration Building at 500 N. Harrison St., Salem, IN 47167 to review and sign the document. You will need to bring with you the following:
1) Student Birth Certificate and/or Custody Documentation 2) Student Immunization Records3) Parent Proof of Residency (current utility bill, lease agreement, mortgage statement, etc.)4) Parent Photo ID
