Loyola High School Entry Screening
Loyola High School is committed to keeping the community safe during the COVID-19 Pandemic. Loyola High School requires all students, faculty, staff and visitors entering Loyola to truthfully respond to the screening questions below prior to entering campus. Please answer these questions no more than 1 hour before coming to Loyola High School. The screening results will be sent to you via email. If you pass the screening process please show your email results to school officials when entering the campus and/or facilities.

1 or more of these: cough, runny nose, congestion, body aches, fatigue, vomiting, diarrhea, loss of smell, OR loss of taste that are not related to a known medical condition
Shortness of breath/difficulty breathing that is not related to a known medical condition
In close contact (less than 6ft) with someone who has tested positive for COVID-19 within the last 14 days
Traveled out of the US in the last 14 days
