AYSO Section 5 “AYSO Achievement -Live Long and Prosper” Award

Overview

• One Former/Current Volunteer nominated per Region and/or per Area can be nominated for this award.
• This will need to be a volunteer from Section 5
• Nominations will be over seen by Section Director Tracie McCormick and Area Director Michael Mays (for whom this award is named for)
• All nominees will be recognized, and an Award-winner will be recognized at the EXPO’s Saturday Award Luncheon in Chattanooga, TN hosted in February 2026. 

Program Procedures

1)     Tracie McCormick SD will distribute these instructions and the “Nomination Form” to all EM's in Section 5.  

2)     Each Region and Area can nominate up to 1 of their Former/Current volunteers for the Section 5 “AYSO Achievement -Live Long and Prosper” All sections of this form will need to be filled out to be eligible for the award. 

3)     RC and AD must submit their form by December 15, 2025.    

4)     The Section Director Tracie McCormick and Area Director Michael Mays will oversee the selection of the former/Current volunteers for the awards.  Nominees & the respected Nominees Family members will be notified by email or letter of their nomination.

5)     Recognition will be given to all nominated former/current volunteers. The Award-Winner will receive a special award at the EXPO’s Saturday Award Luncheon in Chattanooga, TN hosted in February 2026. If the Award-winner or Family is unable to attend the Award shall be mailed to them.
