Explorer Field Trip 
Explorer field trips are ONLY offered on Tuesdays at 9:30 am for 4th through 6th grades.
Only ONE class may attend per day.
NO siblings.
Every teacher must complete this form to schedule a field trip for their class.
A minimum of 3 adult chaperones (teacher + 2 parents/guardians) required.
A maximum of 4 adult chaperones (teacher + 3 parents/guardians) will be accommodated.

Cost per class is $95. Payment by credit card, check or PO.
Field trip date is reserved when payment/PO is received.
