AMS - School Supply Request Form
All requests for school supplies and Chromebooks will be filled as they are made available. Any Chromebook requests made before August 24th will be available for pick up during your student's assigned Materials Distribution Day. For all other requests, Ms. Floresca will contact you to coordinate a pick up time, once the item(s) are available. Please fill out one form per student.

Request for Supplemental School Supplies: Some classes will have materials pre-packaged and ready to distribute for all students enrolled in that class. We also understand that some students may not have all their supplies for the first week of school and may be awaiting delivery. The request for the supplies on this list is for families who may be experiencing extenuating circumstances. Please select what you need. Requests will be fulfilled as the items are made available.
