2018 Forest Glen Vision Screening
The Brown County Health Department staff will be coming to Forest Glen Elementary on October 22nd to provide its annual Vision Screening for Kindergartners, 1st and 3rd graders, and new students to the district.  Parents of students outside of the screening group may request a screening by completing the form below by Friday, October 19th.

Parental consent is not required for students to participate in the above screening. However, if you DO NOT want your child screened, YOU MUST complete and submit the form below by Friday, October 19th.

Note:  If your child does not pass the initial screening, a re-screen will be performed in four to six weeks to determine if there is a persistent vision problem. If they still do not pass, a letter will be sent home indicating the importance of following up with a physician and/or optometrist. The screening is not a substitute for a regular examination by your physician but is meant to identify persistent vision loss in children and bring it to the attention of parents/guardians and teachers.
