2025 - Upper 90 Soccer Camp Registration

Camp Details:
Date: July 14-17 (Monday-Thursday)
Time: 6:00PM-8:00PM 
Ages: 4-14
Location: South Heidelberg Township Community Field 

All player receive a camp t-shirt. Please provide a t-shirt size. 

All players must bring their own ball!

Camp is held rain or shine. If thunderstorms are imminent, please stay nearby. We will try to wait out storm and resume camp when safe. If camp needs to be cancelled one night, the make-up date is 7/18/24.

RELEASE -  *I hereby authorize volunteers/staff of the “Upper 90 Soccer Camp” to administer minor first aid to applicant. In the event that an illness or injury would require more extensive evaluation, I understand that every reasonable attempt will be made to contact me. However, in the event of an emergency, I give my consent for staff to obtain emergency medical treatment. 
  
I/we, the undersigned, individually and as parent(s) and/or guardians of a minor, ask that he/she be admitted to participate in this sports camp sponsored by Courtney Morris & Tyler Witmer. In consideration of admission to this camp, I/we release, discharge, and hold harmless the staff and volunteers of the “Upper 90 Soccer Camp,” and South Heidelberg Township from all causes, liabilities, damages, claims, or demands whatsoever on account of injury or accident involving the said minor arising out of the minors attendance at the sport camp or in the course of competition and/or activities held in connection with the sport camp.
