Howell Athletic Boosters would like to be able to accommodate all athletic programs with the off-season use of the Concession Equipment.  Please read and agree to the following conditions in order to use the facilities:
  • Codes and Keys are not to be shared with anyone not listed on this form.
  • The Named Contact person will be held responsible for the safe use and return of any/all borrowed equipment.
  • All Equipment is to be used as cared for and is the responsibility of the group listed on this request.
  • Any damages or replacement needed are the responsibility of the group listed on the request.
  • Howell Athletic Booster Paper products, cleaning supplies and stock will be replaced if used.  
  • The Facilities are only to be used on the dates requested. (Please fill out multiple requests for Multiple dates.)
  • Only High School Students and Adults are allowed to operate loaned equipment.  
  • If the program fails to follow the Conditions of Use, they will be denied future use.
