Please fill out one form per bag requested (one for you, one for your child, etc.)
Librarian will choose 3-5 books for adults, teens, and older kids requesting chapter books, based off your preferences. Children's book bags may contain up to 10 books. The materials will be checked out for you and you will receive and email once your bag(s) is ready to be picked up.
You will receive a survey each month to let us know how we did.
Book Bags will be available the first week of every month for pickup.
