PCHS Pet Adoption Application
Thank you for choosing to adopt from the Perry County Humane Society. We are a 501(c)(3) non-profit, no-kill animal rescue. Your cooperation and time invested in completing this comprehensive application and questionnaire in its entirety are genuinely appreciated. This allows us to match/place the proper pet(s) with the right family in a much-needed responsible, forever loving home. Please remember, an improper placement or one in which impacting details were not disclosed on an application may end tragically for the new pet. Therefore, please include and verify all information for accuracy before submission.
At PCHS we strive to have a 100-mile adoption radius and are very proud that we are a no-kill shelter. For this reason, if you are unable to keep your newly adopted pet(s) you will be required by law to return the adoptee(s) to the Perry County Humane Society. In the past, we have had issues with newly adopted pets not being returned due to distance and they were unnecessarily euthanized. We want to prevent this occurrence. Even now, unfortunately, not all shelters are no-kill facilities, and 64% of all animals that end up at these shelters are still put to sleep for lack of homes. So if you feel distance will become a factor for compliance (should placement not work out as planned and you must return your newly adopted pet to us), consider please adopting from a shelter closer to your residence.
Remember: completing the application accurately, in its entirety, and considering all involved variables prior to the adoption of your new pet is a must. Adopting is a lifetime commitment. Tail wags and kitty hugs await a loving responsible forever home -- so get that good pen ready, grab a cup of hydration, and -- LET THE ADOPTION PROCESS BEGIN !
For an application to be processed, it must be completed in its entirety. Your signature on this adoption application denotes agreement with the terms set forth as contract with the Perry County Humane Society. Therefore, if you are approved for an adoption of an animal and for any reason are unable to keep your pet(s), by law, you will be required to return the adoptee to the Perry County Humane Society.
The PCHS potential animal adopter understands the inherent risk associated with the interaction and care of animals, including but not limited to the possibility of any injury to person and/or property, including your existing pets, which potential adopters may suffer as a result of the unpredictable nature and/or provocation of animal(s). In consideration of this assumption of risk, the potential adopter and any and all individuals present with the potential adopter hereby knowingly and willingly waives any and all claims, damages or causes of action which potential adopter may have or incur against PCHS - Perry County Humane Society, its officers, directors, agents, affiliates, employees and/or volunteers as a result of the interaction and potential adoption of an animal from PCHS - Perry County Humane Society.
