Thank you for registering for this year's state conference. Please fill out all information on this form completely, as that is the only way to complete your registration. Once you have filled out your online registration, a copy will automatically be forwarded to you from Google. An official invoice and confirmation from Ohio Educational Theatre Association will follow within 5 business days. The amount shown on the invoice should be paid with ONE school check, booster check, cashier's check, or money order. Directions on where to send the check will be included on the invoice from Ohio Educational Theatre Association.
You will receive a separate link for registering students for Individual Events.
As a reminder.....You will need THREE copies of the Emergency Medical Form and Code of Conduct for each member of your delegation. One copy will be turned in when you arrive at conference. The troupe director should keep one copy with them at all times. The third copy must be in the delegate's name badge wallet at ALL times. The copy that is turned in at the beginning of conference will be returned to the troupe director with their packet at the end of conference. In Addition....Troupe directors must read, sign, and bring to conference a copy of the Troupe Director Code of Conduct form. This form requires the signature of your principal. ALL of these forms are available to print or download at http://ohedta.org/opportunities/statethescon/.
Please note that the deadline for name changes via email will be March 2, 2018. Any name changes after that time will need to be made at conference. Name changes made at conference will incur a $5.00 change fee.
Once registered, refunds will NOT be available.
