Please thoroughly complete this form to register your child(ren) or youth into NSUC's Religious Education programs. All children from nursery through high school youth group must be registered. The information you provide helps us create an engaging and supportive faith formation experience in a loving and safe environment.
Visiting families are welcome to bring their children to the program while they get acquainted with us. Visitor forms can be used for the first three visits, after which full registration with this form is required.
You do not have to be a member to register your child. Registration is ongoing throughout the year. There is no registration fee. However, please consider the value of these programs when you determine your yearly pledge to NSUC.
Our religious education program is dependent upon volunteer participation to operate. The most effective way to instill your values in your child is by your actions. You can model the importance of religious education for your child by your own participation and volunteering your time. Furthermore, when more people volunteer, the load is lighter for everyone, and there is more of a sense of teamwork among members. We have paid teaching assistants in the elementary-level classrooms and need three volunteer teachers (each teaching one Sunday per month) for each classroom.
This year we are asking for details of your child or youth's musical abilities and interests. We will share this information with the music staff, so they can invite your child to participate in the various worship services and events throughout the year.
Completing the youth email and cell phone fields gives permission to the church's representatives to use the youth's email address and cell phone number for relevant communications, in addition to the parents' contact information.
1. Practice the Seven Principles of Unitarian Universalism and abide by our NSUC Covenant.2. Members and guests must arrive on time and stay for the entire class/program unless other arrangements have been made in advance with the youth advisors.3. Be respectful to everyone.4. Listen when others are sharing.5. No overt or covert sexual activity and/or other inappropriate behavior is allowed.6. Youth must be registered and active for three months at NSUC prior to attending overnight events.7. Youth may not possess or use tobacco, alcohol, or other drugs on NSUC premises or at NSUC sponsored events held onsite or offsite.8. No verbal or physical violence will be tolerated.9. No weapons of any kind are allowed.10. Certain programs are reserved only for registered NSUC youth group members.11. Guests may visit up to three times. They must register to continue participating after the third visit.
