This permission slip applies to all Sunnyhill (UUCSH) Coming of Age (COA) and/or Youth Group (YG) activities including fundraisers, field trips, service projects, classes, etc.

Please Note: Parents are responsible for ensuring that The Unitarian Universalist Church of the South Hills has the most current information regarding their child. A new Permission Slip must be completed and submitted if there are any changes, including: Medications used, emergency contacts and phone numbers, etc.
