“EASTER FESTIVAL” WAIVER, RELEASE & HOLD HARMLESS FORM. By signing this waiver, I acknowledge that I have read, understand, and agree to the terms voluntarily. I acknowledge that I am fully aware of the danger(s) and hazard(s) associated with participating in the use of the bounce houses, inflatables, and games from The Church of Jesus Christ of Latter-day Saints’ Easter Festival. Participating in this event may pose a risk of serious bodily injury (which occur often), death, or economic loss and I assume all risks. I fully understand this involves jumping on air filled entertainment toys and games and I fully understand the health and safety risks associated with these types of activities. I, therefore, assume all risk of injury and/or death associated with this event.  I hereby release and hold harmless The Church of Jesus Christ of Latter-day Saints and any of its members, against any and all claims, causes of action, and/or any other liability of any kind arising from my participation and activity affiliated with this specific event. I agree to hold harmless those establishments affiliated with this event and accept full and sole responsibility for my action and my children’s actions. I also understand that this Waiver and Release is in force on April 10th, 2020 and covers any and all activities affiliated with The Church of Jesus Christ of Latter-day Saints - Boynton Beach Florida Stake and/or this event. The Church of Jesus Christ of Latter-day Saints - Boynton Beach Florida Stake shall also have the right, make full use, of any photographs, video, or audio recordings made of participants at the event.
