Dance Mania - Studio Rental Agreement
Thank you for your inquiry to Dance Mania! Our brand new facility is equipped with the perfect rental space to hopefully meet your dance needs. Please complete the form below and a member of our office staff will contact you once your rental time has been secured.

* Terms Of Use:  By signing this rental agreement, Renter(s) agree to the following:

* Rates: Hourly (or any part thereof) - $40.00, Half Hour or less - $20.00.
Equipment setup or any other need that requires extra time needs to be factored into the rental quote.

Payment for each rental day is due at time of arrival. Cash ONLY. The date and studio/rental items are not reserved until signed rental agreement are submitted to and received by the Studio.

The rental spaces may be used for dance related activites ONLY.

ARRIVE AND LEAVE ON TIME
• The lease shall start and end as set forth in this Agreement; set-up and clean-up time must take place during hours set forth in this Agreement.
• An hourly charge of $40.00 per hour will be applied for each additional hour that extends beyond rental terms, as set forth in this Agreement.

GENERAL RULES / REMINDERS
• Be present at all times during the rental
• Keep the event or session contained within the assigned studio
• Renter is responsible for proper supervision of minors at all times
• Clean up trash or other items on premises prior to leaving
• Viewing windows must be kept open throughout the time of rental
• Renter and clients must adhere to all current Dance Mania COVID guidelines and policies

STUDIO & EQUIPMENT SUPPLIED:
The Studio agrees to provide the studio in good working order but makes no special guarantees as to said studios functionality or suitability to Renter’s purposes. The Renter is entitled to use the entire studio and any furniture in the studio room at the time of rental. The Studio is not liable for acts out of its control that affect the shoot, such as power outages, weather, emergencies, or Acts of God. Renter agrees to return all equipment, studio, and furniture in the condition it was provided and to immediately notify the Studio of any damage, failure, or change in equipment provided. Rental furniture may NOT be taken outside for any reason.

DAMAGE TO EQUIPMENT, FURNITURE, OR STUDIO:
Renter agrees to pay for any repair or replacement costs of equipment, furniture, or studio that Renter damages within 3 business days. In the event that the Renter’s employees, Renter’s contractors, Renter's clients, or any and all Renter's visitors damage any equipment or studio, Renter agrees to pay for any repairs or replacements necessary within 3 business days. Renter agrees to leave the studio in the condition it was found, or a $100 cleaning fee will be assessed and invoiced.

There is no warranty that rented items are free of defects.

The Renter agrees that the Studio holds no liability for any damage or injury caused by use of rental items to Renter or any third party. The Renter assumes all risk of personal property damage or personal injury. If any accident involving the Studio's rental items has occurred while it is in the Renter's possession, Renter shall make the Studio aware by written statement of details of occurrence of event including police report and names and addresses of witnesses, if applicable.

LIABILITY:
Renter agrees to get a signed Liability Release from all persons that will be on the premises prior to use of Studio. The Studio will not be held liable for any injuries or accidents to the Renter, Renter’s employees, Renter’s contractors, Renter's clients, or any and all Renter's visitors that occur within or outside the studio premises.

COVID Policies:
A copy of our current COVID policies will be e-mailed to you. However both the renter and clients are expected to adhere to the following:
- NO parents or spectators will be allowed in the lobby area for viewing.
- Participants must bring their own sealable bottles of water. NO open containers allowed.
- All participants must either be dropped off at the door or park at the front of the studio.

Use of the studio requires the following in advance:
• Signed Studio Rental Agreement
• Payment
• Signed Liability Waiver and Releases for all persons who will be in attendance at the Studio
• Proof of Insurance
• Proof of Background Check with evidence of no prior charges that could be deemed harmful in any way

The Studio shall have the right to inspect the equipment, studio, and furniture at any time during the rental term. Renter shall make any and all arrangements necessary to permit a representative of the Studio access to the equipment and studio. If a breach of any of the provisions of the Rental Agreement occurs, the Studio has the right to revoke Renter's access to the equipment and studio without any liability to and without prejudice to the Studio, the right to receive rent due or accrued to and including date of revocation. Refund requests must be made in writing and will be responded to promptly by management.

I certify that the information above is correct and true to the best of my knowledge.I acknowledge that if I believe event conditions are unsafe or I am unable to safely perform any activity, I will immediately discontinue participation. I further acknowledge, understand, appreciate and agree that mine or my client's participation may result in possible exposure to an illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my child/ren's participation and exposure.  I further understand the risk of exposure to injury and/or infectious diseases, for myself and my child, as a participant, spectator at events, classes or ourpresence at the facility. I hereby release, discharge, covenant not to sue and AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS Dance Mania Allstars LLC from all liability, claims, demands, losses or damages on the minor’s or my accountcaused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including,but not limited to injury, negligent rescue operations, and/or exposure to infectious diseases and I further agree that if,despite this release, I, the minor, or anyone on the minor’s behalf makes a claim against any of the above RELEASEES, I WILL DEFEND, INDEMNIFY, AND HOLD HARMLESS each of the RELEASEES from any litigation expenses, attorney fees,loss liability, damage, or cost which any RELEASEE may incur as the result of any such claim. I hereby give my permission for any supervisor, coach or other team administrator associated with the Dance Mania All Stars LLC to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment. I hereby waive, release and forever discharge Dance Mania All Stars LLC and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Dance Mania All Stars LLC activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my child(ren) is (are) physically fit and capable of participation in all Dance Mania All Stars LLC activities and agree to not hold Dance Mania All Stars LLC responsible for any injury, accident, or incident that may occur while my child(ren) are present in the facility. By registering my child(ren) with the Dance Mania All Stars LLC, I agree to allow my child(ren) to participate in said event hosted by Dance Mania All Stars LLC, and hereby release Dance Mania All Stars LLC, its coaches, staff, and employees from liability for any injury that might occur to my child(ren) while participating in the Dance Mania All Stars LLC program, including travel to and from activity, organization events or other scheduled activities.I agree to indemnify and hold harmless Dance Mania All Stars LLC, their agents and/or employees, against any and all liability for personal injury, including injuries of any kind even resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) are participating in the Dance Mania All Stars LLC program. I understand that all registrations must be processed through this online portal in order to be eligible for attendance. I understand I have the choice to pay online via credit/debit card or in person using cash or check. I further understand that should I choose to pay online; I will be charged an additional processing fee of 3.0% + $0.30. Should I choose to pay in person, I understand I will not be charged a processing fee. Payment will NOT be accepted without first having completed the online registration portion. I also understand that Dance Mania All Stars LLC reserves the right to cancel or change event schedule at any time for any reason deemed necessary. If course/event that I have paid for is changed or canceled for any reason and there is not an alternate option that fits my needs; I will be eligible for a full or partial refund. If I pay by credit card or ACH (online) I will be refunded the amount of the session or partial session and will not be refunded the processing fee. I will not be charged a refund processing fee if the refund is due to a change brought on directly from Dance Mania All Stars LLC. I understand that if I decide to withdraw at my own will, Dance Mania will decide if I am eligible for a refund. Eligible refunds given for reasons not withstood by Dance Mania All Stars LLC will have the convenience fee subtracted from the amount refunded. I understand that Dance Mania All-Stars LLC produces promotional material about their program. I understand that my child may be included in video tape or photography taken during any activity sponsored by Dance Mania All-Stars LLC which may be published on any social and marketing content owned by Dance Mania All-Stars. I grant Dance Mania All Stars LLC and all other commercial exhibitors the exclusive right to photograph and/or video tape my child and further grant the right to utilize their name, face, likeness, voice, and appearance as part of any sanctioned activity, advertising, or promotion without reservation or limitation. In granting this license, I understand that Dance Mania All-Stars LLC is under no obligation to exercise any of its rights, licenses’, and privileges herein granted.
