Permission and Waiver:
I, the parent/guardian, hereby grant permission for my child(ren) to participate in the art classes organized by Handiwork Studio. I understand that participation in the art classes involves activities that may pose a risk of injury (scissors, staples, etc.) I acknowledge that I have been informed of the nature of the activities and that I accept any and all risks associated with my child's participation.

I agree to release, waive, discharge, and hold harmless Handiwork Studio, its officers, agents, and employees from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child or to any property belonging to my child, whether caused by the negligence of the releasees or otherwise, while participating in the art classes or in any activities related to the classes.

I further agree to indemnify and hold harmless the releasees from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my child's participation in the art classes, whether caused by negligence of releasees or otherwise.

I understand that Handiwork Studio does not provide medical insurance for participants. In case of an emergency, I authorize Handiwork Studio to seek medical treatment for my child, and I agree to bear any costs incurred.
