Vital Information for Consideration

Voluntary consensus-
You are being asked to engage in a research undertaking. Involvement is voluntary. No penalty or loss of gains will be provided in case you make the selection of not participating or discontinuing involvement.
Objective-
The purpose of this study is to see if participants are comfortable leaving their homes while in this pandemic and how they prefer doing daily activities either online or going to certain locations physically to complete those daily activities.
Duration: 10 - 15 minutes
Techniques and undertakings-
You will be asked a series of questions as you fill out a questionnaire. Threats: Certain predictable probable threats or anxieties of your involvement may comprise uneasiness.
Benefits-
You may not directly gain from this study. However, the information gained from your involvement could be of assistance to others in the long term.
Options-
Involvement is intentional, and the only substitute is not to involve oneself in this research study. By checking the box below, participants are confirming that they are 18 years of age or older, able to legally participate in this study, and agree that they have read and understand the details. Participants have had ample time and opportunity to query about the details of the research paper and make a decision whether or not to proceed with the study.
