EGFH & EGFY School Application Form
This form should be completed by any school becoming a member of EGFH (High School) or EGFY (Middle School). Please complete this form in its entirety and direct any questions to your EGF development representative. 

This form will ask:
  1. General information about your school
  2. Contact information for your accounts payable and business office, billing address, and head of school information
  3. Tentative titles you intend to participate in this year
  4. Signature by authorized person to complete the application
  5. Affirmation of understanding the responsibilities of an Esports Director or equivalent
If you cannot complete a google form, this form may also be completed by completing a digital hard copy and returning it by email to your EGF representative.
