Please note that we cannot guarantee blackout requests and that the closer to schedule creation the less likely your submission will be included.

***If this form is submitted multiple times for the same team the most recent entry will be used. For example, if you submit a blackout for November and then a week later you want to add a date for January, when you submit a second time, you MUST include both dates otherwise it will cancel your November date.***
