Short Breaks Survey
Since September 2016, have you applied for or renewed your Short Breaks allocation or are you waiting to hear about Short Breaks provision for your child or young person?

If so, you are probably aware that there have been changes to the way Short Breaks applications are made and provision is awarded. We’d like to ask you some questions about your experience of this new system so we can speak to the Local Authority about how the changes have affected Norfolk families. Your voice matters and we are committed to trying to improve services based on what you tell us.

It would help us if you could answer all questions that apply to you but if you don’t feel happy in answering a particular question, then please leave it blank.

Since September 2016, the assessment of a family’s need to receive Short Breaks has been done by filling in a Resource Allocation System questionnaire (RAS; https://www.norfolk.gov.uk/children-and-families/send-local-offer/care-support-and-advice/short-breaks/resource-allocation-system-ras) The questions in Section A are based on this questionnaire and your experiences of using it.
