Since the COVID-19 pandemic began, the American Chinese community has spent tremendous effort to help relieve the situation.

This form is for the Chinese community to track donations and support against COVID-19. Any Chinese organization/group/individual who donates (materials or funds) or lends a hand during this trying time is welcome to use this form to report their contributions.

However, please only submit a record if you send it directly to the beneficiary in order to avoid duplicate reporting. For example:

* If you personally donate 2 boxes of gloves to a nearby shelter, please submit the record here.
* If you donate 100 surgical masks to your church group, and the church group collects and sends all the proceeds to a local hospital, then please let your church group know to submit the aggregate records using the form.
