As we prepare for the possibility of returning to in-person instruction, it is important that we have information that is needed for our planning process. This will assure that we are able to fulfill the requirements necessary to resume in-person instruction. Please answer the questions below. If you have more than one student attending Sierra, PLEASE COMPLETE THE FOLLOWING FORM FOR EACH OF YOUR SIERRA STUDENTS.
